We are currently seeking a Care Coordinator in our Home Health division to be responsible for ensuring a positive patient and family experience through effectively scheduling direct care providers in the community to meet the requirements of patient care plans. Care Coordinators work diligently to provide as much consistency of care as possible while meeting the scheduling requirements of the patients and care providers. Frequently problem solving, they are the central point of contact for patients and families, care providers and referral sources. Effective care coordination is an important element of service delivery. The individuals in this role must be driven to help others in a compassionate and caring way while balancing the pressures to deliver on scheduling requirements.

    Care provider scheduling and service delivery coordination

  • Effectively utilize of scheduling software to ensure patients care plans are fulfilled with appropriate care
  • Communicate effectively with care providers to ensure schedules are carried out (avoiding missed visits, unfilled visits)
  • Provide field staff with accurate schedules on a weekly basis
  • Ensure care providers are scheduled effectively and efficiently (allowing appropriate time for travel between visits, ensuring visits are scheduled in contained geographic regions whenever possible)
  • Communicate effectively with patients and families when schedule challenges arise; problem solve effectively

  • Employee and Patient Relationship Management and Support

  • Maintain accurate records of care provider availability
  • Ensure continuity of care whenever possible
  • Seek to understand the individual needs of patients (and families) and care providers and consider these needs in decision making whenever possible
  • Maintain accessibility by phone for patients, care providers, referral sources and other stakeholders
  • Actively pursue positive relationships with care providers and patients
  • Follow up appropriately on missed visits
  • Liaise with Care Supervisors to share patient and care provider feedback
  • Resolve or escalate as required client concerns/issues

  • Documentation/Administration

  • Dated notes are timely, clear, contain appropriate and required/necessary information
  • Documentation and/or phone calls both externally and internally are accurate and timely
  • Responses to faxes and emails are timely
  • Required reporting or tracking is completed within expected timelines

  • Quality and Risk

  • Enter all critical and relevant information into the system for appropriate follow up and documentation
  • Daily performance and tracking reports
  • Escalate appropriate concerns or risks for review and/or follow up


  • Ability to work independently and also as an effective member of a team
  • Excellent interpersonal and communication (written and verbal) skills
  • Customer Service experience
  • Strong Computer skills
